Floreat Surgery has serviced our community with high quality medical care for over 30 years.
In July 2021, four local GP’s from Chandlers Hill Surgery purchased Floreat Surgery to ensure services would continue to be provided.
Our reception and nursing staff proudly support 6 Independent Contracted General practitioners who continue to provide comprehensive medical care to our patients living in the southern and surrounding suburbs.
We are part of the RACGP training program and provide teaching and training support to GP Registrars.
Due to Medicare rebates no longer being a sustainable operating module Floreat surgery encourages mixed billing and GPs may no longer bulk bill patients to Medicare routinely.
Consultations are made by appointment and are 15 minutes per patient to assist with 1-2 concerns.
Longer consultations for multiple problems, insurance and employment medicals (charge apply), removal of lesions, first antenatal visits, and full “medical” checks can be booked for 30 minutes.
On arrival please report to the reception desk and our staff will check your identifiers by asking for you full name, date of birth and address.
Please advise us of changes to your address, phone number including mobile, Medicare or concession entitlements and next of kin.
Test results, recalls, repeat prescriptions and referrals require a booked appointment.
Telephone consultations are available, if medicare eligible, by booking only, fees apply.
New patients require a 30 minute appointment with the nurse followed by 30 minutes with the GP.
Urgent matters e.g. chest pain, trouble breathing, feverish child, and allergic reactions need to be dealt with as a priority and may effect your waiting time. We apologise for any inconvenience.
If you are unable to keep your appointment, please notify us as soon as possible so that the appointment can be offered to another patient in need. Failure to notify, may incur a fee.

Telephone Consultations
Telephone Consultations are available with a Doctor, only if you have visited the clinic for a face to face consult in the past 12 months, as regulated by Medicare.
All telephone consults for scripts, referrals and results require a booked appointment and will be scheduled in the next available time.
Home Visits
This service is only offered by doctors for current eligible patients, on a case by case basis.
Aged Health Assessments are available to patients over 75yrs which involve a nurse attending your home and then following up with a GP in clinic.
If you are interested in this service please call the surgery to discuss.
Cancellations
We would appreciate you telling us as soon as possible if you think you may be late or need to cancel your appointment. Sometimes your appointment may need to be rescheduled if you are late.
A cancellation fee may apply for not attendance at appointments without sufficient prior notice (normally 2 hours).
Translator Services
If required, the practice can arrange a translator/interpreter through the translator service or the national Auslan booking service.
Please advise reception staff if you require this service.
Patient Feedback
Our practice endeavours to provide a comprehensive high quality service for our patients. At times we may invite you to complete a questionnaire to identify areas in which we may improve that service.
If you are unhappy with any aspect of our care please feel free to discuss this with the practice manager personally, or by phone, or letter.
However, if you feel there is a problem you wish to take up outside, you may wish to contact the S.A. Government centre for handling medical complaints.
